Title: Code to display three columns

I've modified the plug-in and I do not succeed on the ad insertion page to put the options on three columns.
I want to put on three columns because there are many options.
How can I use collapse to hide check mark options if the user wants to expand whether or not.

I managed to modify and successfully change the plug-in and now it goes right

I can not show the collapse function that would look better on the page  ???
I tried everything I know and found on the net, but it changes my style page and I can / do not know how to correct it.
