Help - Need to add city search box

Hello Guys :)  ;)
hope all doing great ,
i tryed to edit inc.search.php and functions.php to add city in homepage box search but lock like too hard for me
look at this picture



what i want just show only categoty and region and city  :-[

i'll attch my search.php , inc.search.php , fucntions.php in my theme
and some one edit it for me  ::)
