Help please..How to create a credit application form on osclass ?

Hello i need a little help if possible i have been trying to contact osclass for at least 6months now sent tons of emails not one reply from them..
i am trying to create a site credit application form with file attachment and submit button on my website  using osclass  next theme is there a plugin available  or  video instructions any information that can be useful to making this possible have searched and cant seem to find what im looking for any help would be appreciated..
